Charting Physical Literacy Questionnaire

Dear participant,

Greetings, we appreciate you taking the time to complete this research-based survey which aims to chart your physical literacy status. With the support from Chinese Physical Literacy Association and International Physical Literacy Association, the Sports and Society Research Team at The Chinese University of Hong Kong established the Charting Physical Literacy Questionnaire.

This study aims to explore compelling impacts on an individual’s physical literacy journey through a reflection on their physical activity engagement throughout their life, as well as to analyze the impact of physical activity on individuals' overall health. 

The survey is conducted anonymously and your responses will only be used for academic purposes and be treated strictly confidential. This survey could be finished in approximately 10 minutes. There are no “right” or “wrong” answers. We would like to know how far these statements match your personal view. Please do not leave out any questions.

This survey will carry out a validation process and build a greater understanding in related to your demographic information, physical literacy, and support for the development of active environments for future as well as present generations. I consent to participate in this research project.

● I understand that the purpose of this survey is to develop a self-report scale to assess the Characteristics of Physical Literacy Journey and have also been provided in a language that I can understand. 

● I understand that I will be required to participate in a web-based survey at a time most convenient to me. 

● I understand that my participation is voluntary, and I am free to withdraw from this research anytime without needing to provide any explanation, and I would not receive any penalty nor bias as a result of my withdrawal.
